I confirm that I am an Accredited Counter Fraud Specialist and I consent to this being verified with the University of Portsmouth.
 
I confirm that the information I have provided is correct and complete to the best of my knowledge and I consent to the Institute of Counter Fraud Specialists using my contact details and photograph for membership purposes, and any other purpose considered appropriate by the institute.
 
I confirm that if I am ever convicted of a criminal offence involving an act of dishonesty I must inform the Secretary of the ICFS within seven days, and I understand that the Executive Committee will make the ultimate decision on my membership status.
 
If found that I have breached the ICFS professional rules of conduct I accept that membership will be terminated by the ICFS within seven days of the breach being identified.
 
I accept that my membership will be terminated if any information I have provided on this form is incorrect.
